We operate a contenental type shift pattern over 7 days a week but the shifts are all 12 hour. I have been looking for a template for a risk assessment for shift work and can't seem to get hold of one. We offer the health screening etc and have measures in place for any employee issues relating to health and shift work and also have a policy in place but no actual document for a risk assessment. The reason for trying to find one is that when one of the employee's suffered with stress issues I have been asked to do an assessment for him to return to his shift pattern eventually/over time and I just thought that it seems pointless having something in place for a temperory issue if we have nothing for a permanent pattern.

There is no such thing as a good shift pattern as they all have risks. However, some patterns are better than others.
When I evaluate shift patterns for clients I tend to use the 'good practice guidelines' listed by HSE at http://www.hse.gov.uk/humanfactors/topics/good-practice-guidelines.htm. I make a judgement of how well the pattern complies with each of the points to highlight where it may fall short.
To convert this to a risk you can refer to the "undesirable effects of shift work" listed as paragraph 18 in the HSG 256 - http://www.hse.gov.uk/pUbns/priced/hsg256.pdf
One thing to watch out for is that workers often work to a different pattern due to swaps, overtime etc. This needs to be controlled, but individual risk often different to the 'expected' risk as per the defined shift pattern.

Sounds to me like you require an individual stress RA for this employee. You ought to do this to identify the stressors that are affecting the employee and what actions can be reasonably taken to reduce the risk of triggering stress. Change in role, responsibilities etc.
