I work in a busy Transport site, which is not owned by the company.  They have been unwilling to invest in solutions to seperate or segregate pedestrians from traffic.  Marked pedestrian walkways are limited and there is no clear traffic management plan for the site with shared entrances and parking/site vehicle movements.  I am even struggling to implement mandatory PPE for staff/visitors on site.  Is anyone aware of any examples of HSE prosecutions for failure to utilise PPE?  I want to use evidence based arguments with the directors to get site improvements implemented as a matter of urgency.

What PPE are you wanting to implement and why?

PPE should be considered a last resort.

There are loads of prosecutions out there - try looking on SHP on-line, there was a recent on in May which describes more or less the same situation that you have.  I would expect everyone to be in hi-vis and safety footwear, not that a safety shoe will give you much protection from a vehicle but because you tend to get odds and scraps of debris around and you need a shoe with a decent sole.   Here is your starter for 10 - loads in this list!

And where is this mandatory PPE to be situated? Not effective if it is in a locker on the other side of the yard from where people arrive at the premises - which is one of the reasons PPE should always be last resort.

You need to be pursuing traffic segregation failure prosecutions not PPE issues, even if I were wearing every possible piece of Hi-Vis as I exited my car if you aren't adequately segregating people from traffic that is where the prosecution will come from.
