Hello,

I work in a manufacturing sector and recently met with an accident involving the collapse of a quarry face onto an excavator with only material damage.

I need an advice on the correct procedure to be adopted when using excavators (equipped with jack-hammer) for de-rocking actitivites.

There is some useful information on the HSE's website regarding quarries and the type of equipment. A lot of it boils down to height, reach and safe distance of the machine. Does your geotechnical assessment identify failure planes/mechanisms for rock fall. Consider rock traps, placing the cab postion outside of the potential failure plane of rock fall and engulfment etc.

You should also be consulting with a geotechnical specialist to idenify potential issue areas, particularly when designing in traps as you will need to also consider stability of the surfcae the plant is sat on, access and egress widths etc. when implementing your safe system of work.

For the moment I don't have much of procedures. I have just started up building the risk assessment and SOPs.

Sounds like they may have undermined the face and gone it too far before taking the next layer off or the machine is too close to the face with its reach both need looking at to prevent it occuring again. The face is likley to fall due to machine actions onto the boom if its a pick type system but not onto the cab area
