Dear Members
I have been a Graduate Member for years now and I was interested
in doing my IPD I have my NVQ5 and NVQ 6 in Health and Safety
I would be grateful if someone could give me a guide on the
questions and the scenarios before I sign up for the route.
This would really help me
Thanks

It will be dependant on how you reached Graduate status to which Chartered route you take. It would be beneficial to give membership a call and have a chat with them, they will be able to advise what route is required.

Ring IOSH Membership, pay your IPD fee (approx £160) and you will be enrolled. You'll have 2 years to complete the elements of IPD - which in your case is likely to require completion of the Open Assessment (examination), CPD Audit and then the Peer Review Interview. Details of the Open Assessment can be found on the membership pages of the general site.
On signing up etc - IOSH will provide you with an IPD pack that will offer you guidance and support on getting through the process. In terms of questions, what the Open Assessment entails, the PRI etc - the info you receive from IOSH will cover this.

upon assessment A I believe it is multiple choice, if you fail can you resit?
if you have 2 years what is the 14 days situation all about?
sorry to sound vague.

14 days is the time from which you 'start' the Open Assessment - through to needing to submit your answers. The 2 year period is the time from which you enrol on the overall IPD programme through to overall completion (OA, the CPD Audit and your PRI).
(The 2 year time limit may have gone? - but I can't imgaine IOSH would want members sat with open IPD accounts for more than 2 years.)
