I don't like to be lazy I have already searched the forum and not found any info on this top.

Most of my career has been in manufacturing, so for me.

If there has been an accident whereby a causation has been gasket has blown due to a lack of preventative maintenacne I'd say that would be systematic failing?

On the other hand an operator got caustic in his eyes because he was not wearing PPE and the culture of the site was no consequence for wearing PPE I'd say the cause was systemic?

Would people agree?

Are there any tools recommended for systemic and systematic analysis?

Hmmm I don't think you have to have a consequence, in particular a negative one (punishment?) in fact the lack of consequence does not actually set out the expectation. (ABC). 

The systemic failure refers to the failure of the whole (process, department, organization etc). In this case could be that the expectation that PPE will be worn is not made clear which could indicate a systemic problem. 

To be fair to the workers you can't have a consequence for not complying with something you didn't know you should have been complying with.  So systemic failure is a little deeper than having no consequence, you have to set the expectations first.  If management want the work done but don't tell the worker how and/or don't invest in making sure it can be done safely this might indicate a systemic problem

The gasket blowing could be a systematic failure (it’s a predicable failure due to a known or certain cause, the lack of maintenance).  With the lack of maintenance being causal, the reason for the lack of maintenance could be systemic......................
