I am using Croner to record accidents/incidents but would like to work up something for our staff to better report accidents/incidents through, rather than via paper or email. Does anyone have any systems they are using or systems they have worked up?

I'm not really sure what you mean - something that is easier than paper or email, not sure what sort of options you are looking for.  Can you give a bit more information?

Hi Andrew,

We're in the process of moving away from Croner. So far we have two front-runners, one of which is quite specialised, but they are both modular, web-based, fully configurable and most importantly have a configurable workflow which means that people who need to know get informed when an incident is reported. They also have a full suite of reporting and incident investigation/management tools. The more general system is Alcumus,

John

We had our IT department build a system for us which is ever evolving. We issue every employee with a user name and password and the system is available on 99% of terminals around the site. Our reporting of near misses has gone up around 150% since we implimented the system. Accident reports are done virtually straight away and the relevant people informed straight away. I am happy to discuss the system if you wish and our IT department is a stand alone company so they would be happy to speak to you about it. I am not in any way trying to sell anything, but if I can help let me know.

It depends on what you want from your incident reporting system.

You can easily design an incident report form on SurveyMonkey or similar survey tools and publish a link to it.

Years ago, me and an IT colleague knocked up an incident reporting system using Access. It was exactly what I wanted-not pretty but functional and it particular it created data that I could analyse.  Nowadays I suppose that are other packages that would enable you to create such a system from scratch. The problem with a bought in system, is that you often end up having to modify your procedures etc to accommodate what the system wants, rather than the other way around.  Think about what it is that you want it to do and how you want to use it before deciding what to go for. Don’t let the vendor tell you what you need to be doing.

My Company uses a system called Safeguard for reporting of all incidents and has an option for starting an investigation and raising the actions on the same system so a 1 Stop Data point if you like.

I haven't used anything else so can't say how good or not it is against other options out there but you do have some scope to have changes made to make it more reflective and user friendly.  I am sure a quick a google search will bring up many options and hopefully you will get some feedback on them from other users

Good luck
