We are really pushing to get more proactive reporting within our workforce, as well as the contrcators on site. We tell them the importance of reporting unsafe conditions and acts on a regular basis, but unfortunately it's basically only observations from us HSE guys that are in the system.

Has anyone had any success in increasing the numbers of reports/observations after implementing a certain system? e.g. an incentive scheme. I would be interested to know specifically what, so we can try it. We have used a small prize for the best observation on a montly basis month previously, but it wasn't effective.

Don't like the idea of a prize. Try a small donation to the charity choice of the 'winner'

Make it so easy, so simple that they can't resist, verbally, by phone, text, WhatsApp, whatever.  They give the info but you may well have to put it into a recordable format.  Often the unsafe act is momentary and the time/ effort to report it by the observer is way too high.   ther is as well as experience) that some tend to report the unsafe acts they commit but tend not to "tell" on the unsafe acts of others, even if they were very unsafe acts or repeated minor ones.   S1a1
