I am looking for a good Health & Safety survey that is directed towards Senior Management/Leadership.

Whilst a vital component in working towards a positive Health & Safety culture we have all I am sure experienced different levels of ownership and involvement at Senior Management/Leadership level

Has anyone used a specific survey that they could please recommended?
