I know its a long shot but does anyone have a pdf or hard copy of the above leaflet?

I called the HSE who say its now only available from the SIKH COMMUNITY & YOUTH SERVICES who I've tried calling but there doesnt appear to be anyone in as the phone keeps ringing and ringing without answer.

Any help would be appriciated.
