I am looking for further details on the above inquest. It involved a boy who was killed while on work experience on a farm. I have a brief summary of the findings from the DfEE, but I was looking for a little more detail.

Any help would be apreciated. Thanks, Ian.

I attended most of the inquest and may be able to help.

What in particular did you want to know?

Yes Ashley, this was the unfortunate incident where a 15 year old boy died in a grain silo while on work experience on a farm.

I was basically looking for further information surrounding the inquest. ie How the accident happened, vetting procedures, pre and de-briefing involved etc. prohibitions set etc. and whether a successful prosecution was brought against any of the parties involved.

I am involved in setting up our own work experience scheme from scratch and I just don't want to repeat any mistakes previously made.
