Can anyone tell me where I can find a CDM F10 notification form on the net.
I've tried HSE direct but the scanned version is pants when printed. The HSE site does not appear to have one either.

I will be interested to see if anyone comes up with a site. When faced with the same problem, I found it easier (more convenient) to call the HSE and request that they send me some paper copies - which they were happy to do. I have since copied the originals as/when necessary, using good copiers to control the quality.
