Hi,
I work for a PCT and I am currently reviewing the incident reporting system and I am having lengthy talks with Clinical Governance. I should be very grateful if anyone could let me know how their system works, i.e. where do the completed incident/accident forms go to first, who gets copies and within what time limit, who's responsible for RIDDOR reporting etc. We have just purchased Safecode, but have yet to install the package. This information on what other Trusts do would really help with my discussions.

In our Trust all the signed (by the manager)completed forms (and there have been problems with uncompleted forms which are returned to the sender)are sent to the risk managers where the details are entered on the PC. The manager should keep a copy of the form sent off and I as the COSHH Advisor also get a copy (otherwise I have to wait a while until the forms go though the system - which is no good if an incident wattants an investigation by me as well as the dept. manager). After they are entered, copies of the forms go to people like Occ. Health, the manual handling advisor(or he regulary goes across and reads his pile)me etc.(if they've forgotten to send me one. There are boxes to tick as to who receives this info.).We no longer keep accident books. Staff who fill in forms are usually encouraged by their union to retain a copy for themselves.
As for time frames, the manager has to send the form in within 5 working days (allows for witness statements to be completed and attached to the form)and the processing though the risk manager's dept. relies on whether the staff are present or ill(think reality in the NHS). The risk manager's office is linked to the litigation dept.but not to H&S (NHS politics in this Trust)
RIDDOR forms are completed by the dept. manager, sent off and copies retained.
Suggest though whatever you do, put a load of training in with your managers.

in my trust all incident forms are completed at department/ward level and are sent to the secretariat. They are responsible for inputting the information and also for notifying any RIDDOR incidents. Anything that concerns any of the specialist advisors (h&s,fire,man.handling, infection control) is then flagged for investigation and comment.
We are currently in the process of changing over from SHE to Datix. Once this is complete it should cut down on the paperchase (if that is possible in the NHS)as the info will be accessible from more pc's.
