Posted By gavin leatherbarrow I am looking to introduce a standard form that can be used by staff which covers accidents and incidents etc. Could anyone point me in the right direction of a website or publication? Any help would be most aprreciated!!

Posted By Richard Gavin I use separate forms for accidents and incidents since I find the information requirements differ somewhat. You're welcome to a copy of each, but bear in mind they're FE orientated Richard
