Andy,

I would like to suggest that when accidents are investigated, in particular those which are more serious and therefore more likely to be the subject of an enforcement agency visit, that care is taken as to how the investigation report is put together.
I regularly represent clients where there are real difficulties in defending prosecutions because of the report findings.
