You may try Health and Safety Games for Trainers by Graham Roberts-Phelps published by GOWER ISBN 0 566 08202 0 which contains True/False Quizzes, Knowledge Tests, Self-assessment Questionnaires, Workplace Assessment Questionnaires, Ice-breakers, Group Energizers, Safety Case Studies and Legislation Discussion Groups and Crossword and Word Search Puzzles.
You may also find it useful to try the search facility on this forum, as this subject has come up before. Try typing in "quiz" in the keywords and hopefully the archive will identify a few ideas.

Just an idea that might be some fun.
A couple of years ago my organisation were holding a h&s conference and the same topic arose. The problem was that some of those would have a better knowledge of safety than others, therefore, for some the questions would be rather easy.
I suggested instead of a quiz we could have a picture with various hazards inserted and the participants would have to identify all the hazards. Guess who got the job! The setting was an office, (you could use a hospital ward) which I drew in perspective and filled with various office furniture and equipment. I obtained most of the 'scenery' and hazards from a 'clip art' file. Okay, the the dynamite under the computer desk was a bit obvious...
