Dave,
If you tell us what you think a complete system contains, we should be able to help with examples. For instance, I have a suite which includes hot work, electrical, general and confined space formats which may help. But in my experience permits are often wrongly used to attempt to try and handle situations which require more physical controls.

Can you give me a bit more information re what you require? Is it management systems or specific permit to work systems? This is a massive area that requires plenty of thought before implementing and after all will be operated by the most unreliable bunch....people.

Totally agree with Bob and Alan, however I can offer a procedure and some permits in Excel form that after editing may suit. Also it would be wise to risk assess the area you require a permit for, and using the information derived from this during editing.
