Alan,
One that I always use is called 'You always do your best' and was given to me by Powergen at a health & safety seminar they ran some years ago.
I don't know whether they still have any copies but it may well be worth contacting them (if they still have an occupational health & safety unit!!!).

I sometimes use a training pack produced by a 'Health & Safety productions' called
"How did it happen? "
An advantage of this video is that it is in the style of a murder mystery and can be made participative as role play scripts etc. are included.
It comes with a trainer's guide and is useful for getting the message, not to jump to conclusions across.

I've used the undernoted video when delivering Accident Investigation Training.
Company usually allows preview before purchase. Several accident scenarios presented with stop/start audience participation in question & answer sessions, includes forms printable from CD-Rom version.
I have no vested interest in this Co. or it's products.
