Posted By Angela We are considering moving away from our current A4 report form for hazards/near miss reporting and introducing "pocket size" cards. Does anyone have examples that they would like to share, so I can take the best ideas and make it appropriate for this site. Thanks in advance.

Posted By Sarah J Shaw Angela I use an A5 duplicating form which I send you if you e-mail me directly. We call it an "Unsafe report". People couldn't get their heads round near miss definitions and the size of the form put them off reporting. These A5 forms are easy to use and I still make the supervisors investigate every one.
