I would be interested to know if there are any qualifications necessary to carry out risk assessments on hazardous substances. If not, have any individual companies set up their own scheme to judge competence and give training for such risk assessments?

No qualifications are needed to carry out COSHH assessments although such qualifications can be useful in establishing the competence of the assessor (as long as they have the skills and experience, as well).

The words used in the COSHH ACOP are that the assessor should have "adequate knowledge, training and expertise in the assessment, evaluation and control of risks arising from exposure to substances hazardous to health".
It suggests types of training course suitable for when formal training is required.
