Posted By Frederick George Robotham I have been trained in & have used the Shell Tripod and M.O.R.T.methods of accident investigation,have read Ted Ferry's books on the subject.I keep coming back to the Australian developed Accident Reference Tree-Trunk method of investigation as I think it is the best. The systems that I think are best are:
National Safety Council Accident Investigation. This is a good starting point for supervisors and others for general accidents.
MORT cut sheets and TOR for closed systems where you have a lot of politics; it forces them to ask and answer questions.
Appollo Root Cause analysis and STEP for open investigations.
