Posted By Malcolm Hogarth I am looking to set up a near miss reporting system in a food manufacturing environment. Rather than re-invent the wheel I wonder if any of you kind folks out there who have done this already would be able to assist in providing me with some hints and tips on the best way to go about this - perhaps with examples of the form that you use.
