Does anyone have a manual handling risk assessment form they would be prepared to let me have a copy of?

I Have two examples but there not user friendly, so I'm trying to design one that is.

Agree with Peter, above, the example given by the HSE, in the ACOP, is as good an assessment form as you need and you are unlikely to be censored by the inspectors, when audited, if you are using their own assessment form. I use this form when carrying out our MH assessments but I also incorporate lots of photographic information, diagrams and plans. If you would like an example give me a call.
