I am a Back Care Advisor within the NHS. The current manual handling risk assessment form we use, which contains a numerical matrix score, is not user friendly, and most staff members have difficulties in completing them, or more often than not, not completing them.

Can anyone forward me any assessment forms they have, that may be of use, or that I could adapt into the current forms.
