Posted By Antony Ferguson Does anyone know of any recent cases of unauthorised access where equipment was not securely locked off/ prevented from being entered. I'm trying to convince our management of the need to prevent open access to process vessels, etc, where there are hydrocarbons, flame, heat, etc, for vessels that have historically have never been locked off. I'm in need of a bit of evidence of what happens when things go wrong,eg, Uddin v Portland Cement 1965.
