Posted By Lisa Eldridge Hi This may sound like a strange request but does anybody know of any websites that I may be able to get some examples of incidents/accidents and even fatalities involving hazardous substances. I am carrying out some training and would like to include some examples of when it can go wrong. I work in the health care industry so articles relating to this area would be more suitable.
