could anyone tell me who is allowed to fill in the accident book and refere to it at a later date i am the hs rep and first aider

Anyone can make an entry into the book.
However, after the entry has been made the page with the entry has to be removed and kept in a "safe place" under the care of a "nominated person"...only said nominated persons can read the details within.
It's called data protection, and is a legal requirement.

Over and above that stated above:
The employee can have a copy of the accident book at a later date.
Any other person can have a copy of the full accident book entry if the injured person provides signed consent.
Any person can have a copy of the accident DETAILS as long as it is not possible to identify the person who has been injured (i.e. the injured persons name, date of birth, job title, department? address etc are all taken off the copy given out).
If witness details are listed then these details can only be given to the person who was the witness unless signed consent is given for that to be passed to a third party.
If you think data protection is painful wait until Public Disclosure legislation comes out in January!
