Posted By Rob A client asked us to complete their incident report form. The incident related to nothing more than a cut finger which was not job related. it makes me wonder if reporting even really minor incident is worth it. It costs money and adds to loss. This is the (negative) Root Cause part of their form which I have to decide fits best. Is this really good for client contractor relations?: PERSONAL FACTOR:1 LACK OF SKILL OR KNOWLEDGE 2 CORRECT WAY TAKES MORE TIME AND/OR REQUIRES MORE EFFORT 3 SHORT-CUTTING PROCEDURES OR ACCEPTABLE PRACTICES IS POSITIVELY REINFORCED OR TOLERATED 4 IN PAST, DID NOT FOLLOW PROCEDURES OR ACCEPTABLE PRACTICES AND NO INCIDENT OCCURRED JOB FACTOR: 5 LACK OF OPERATIONAL PROCEDURES OR WORK STANDARDS 6 INADEQUATE COMMUNICATION OF EXPECTATIONS REGARDING PROCEDURES OR STANDARDS 7 NADEQUATE TOOLS OR EQUIPMENT

Posted By Ken Taylor Isn't there a factor marked 'Other' and a space for you to add something else? On principle, it's a good thing for those working in a client's premises to report incidents that may have implications for the client's undertaking - but this needs to be on a reasonable basis.

Posted By Rob Answer to Ken---simply, "no". I enquired, the response being ---"Please note that there are certain protocols in place with regard to the reporting of Incidents and Near-Misses that occur on our sites.Please study them carefully and use these forms and follow these procedures in future!". Which means to me that the client doesn't know either!!!!!! Rob

Posted By Merv Newman Rob, to really understand this accident (and you have already pinpointed the causes) you should talk to the behavioural safety guys. (like fats in a previous existance, though he may wish to deny it now)
