Does anyone have any information/guidance relating to the legal(?) requirement to complete a 'form' prior to carrying out any work at night.

Obviously there would be risk assessment involved but I am led to believe there is a particular form which needs to be completed. Any help would be appreciated.

not sure about any forms in particular, but you should be aware of the working time directive and special conditions for night workers.
They are entitled to routine medical checks and health surveilance.

The form you're asking about is probably the health assessment mentioned in other replies. This is commonly applied as a self-assessment questionnaire. If this is used as a trigger for further examination if a problem is declared it would probably satisfy the requirements of the WT regs,
