We are a chemical manufacturer and wholesaler so the jobs would entail either order picking and packing or manufacture or filling of products comprising jsut about the whole range of COSHH risk phrases apart from carcinogens and mutagens. Any advice on what kind of health questions we should be including on the application form?

In addition to the above, I would have though that arranging for a pre employment eye test would be a good idea, not only because I expect the work will involve DSE, but I as I recall a lot of the info on paper and COSHH substance containers can be in pretty small print!! Also, a form requesting the propsective employee to provide a signature on a request to contact their GP and get details of recent illnesses/time off work on the sick... MSD as mentioned above, and deratitis problems also... Some companies do sent out a long form... asking have you ever had any of the following.... and list just about every ailment you could think of... Stuart
