I have been handed a copy of "Risk Assessment - A Practical Guide", which was published by IOSH as a supplement to SHP, in May 1993, and updated in 1996. The author of the guide and the supplement was Brian M Kazer.
Has there been a more recent update, or new version of this publication?

No, there has never been an update, and in fact the publication hasn't ever been included in the list of free IOSH guidance - as you say it was oringially issued via the mazazine.
Our policy on free publications is that they are developed by Working Parties reporting to Technical Committee, which I currently chair. We try to identify 'gaps in the market', rather than to repeat what is already available. In the case of Risk Assessment, my personal view is that HSE's '5 Steps to Risk Assessment' is rather simpler and at least as practical as Brian's very early guidance, and there is other authoritative guidance readily available, in as much detail as anyone might need (for example in the priced IOSH publication 'Risk Management').
Technical Committee is happy to receive ideas on gaps we should seek to fill - our current plan includes regular updates of existing guidance, plus new publications on Consulting good practices, investigation of serious accidents/incidents, good practice in OSH training, what is 'reasonable adjustment' in terms of DDA? - so we have plenty on our plate, given that all the autors are unpaid volunteers! But still happy to have further ideas.
