Posted By Shamshad Bahadur Dear All, One of our senior management person from project side believe that accident investigation is primarily line management duty and a safety professional comes next to line manager so far as accident investigation is concerned. He also says that this is the standard practice of PDO Oman also. Please do advice, how correct he is? Regards.

Posted By David Bannister In almost all cases, accidents are due to failures in management systems; the aim of investigation is to identify those failings so they may be addressed. The method of discovering those failings should be determined by the organisation and its culture: it all depends upon the motivation, skills, experience and ability of managers whether they are best placed to investigate. In my opinion, line management should take responsibility for accident investigation, supported by safety professionals, although if the organisation does not favour that approach then the safety professional should lead. In short, there is no "corect" answer - it all depends who will do the best job.
