Where a CoSHH assessment has been carried out, does this have to be a stand alone document?
Can the relevant information be included with the risk assessment for the overall task, of which use of a chemical forms only a part?

It can be done either way, its not prescriptive.... saying that it is easier to do it as a stand alone document in view of more than one substance used in a process..... then make refference to the COSHH assessment to the risk assessment
hope it makes sense?

No presecriptive method. Sufficient discussion of all task/process risks associated with storage/transport/use of hazardous substances can satisfy assessment as required by both COSHH & MHSWR on one document.
Avoid bureaucracy/unecesary duplication of paperwork!
