Posted By Robert. We are a small company approx 120 strong and are predominantly field based. We are trying to establish a formal RCA that would be relatively easy to follow. A previous thread hi-lighted the HSE website, as a study was carried out on the many varied styles of RCA. We appear to be going along the route (although we didn't know what it was called) of the Systematic Cause Analysis Technique which we believe to be correct. Is there a "UK" or EU industry approved or recognised standard for RCA available, as the list of acronyms and styles all generally originate from the USA. Or is it a case of "what works best?"
