Sorry Kevin; but because of the range of PPE and the extremely variable circumstances under which it likely to be used there cannot be a simple standard form that fits all.
What PPE in what activities?
In which is it in relation to = specification, selction, use, maintenance, replacement or disposal?
We may then be able to provide some rather more focussed help.

PPE is a control that results from a risk assessment. Having identified the hazards and the necessary controls, whicj may or may not include PPE, ask yourself, what harm is the PPE designed to prevent, then acquire and issue the appropriate equipment.
If you are unsure as to what standard of PPE is appropriate for the protection of your employees, provide the details to the supplier or manufacturer and seek their recommendations.
