I am looking for an age audit to enable me to evaluate my company's attitude to age discrimination. Lack of success is causing me real stress and a lack of motivation.

Can't pinpoint a specific guide to this matter - you may be best off devising your own own survey questionanire.
Assuming you are familiar with the distinction between 'direct' and 'indirect' discrimination, and the concept of adverse impact, it is easy to frame questions about 'direct' discrimination. A good source of relevant information on the issues about content is: 'Aging and Work' ed. M. Kumashire. Taylor & Francis, 2003.
Regarding indirect discrimination, you face the difficulty that your respondents may not really know they are doing it. A good way to design fruitful questions is to enquiry into diversity management. A good source of relevant information on this is 'Diversity Management', R Kandola and J Fullerton,CIPD.
