Hi

Just wondered if any kind sole could share a copy of a attitude survey to help me measure employee perception. A scored system would be advantageous however any are greatly received.

This will be tested on a company workforce that up until now are very dismissive of safety.

I must say that this is basically evidence gathering for a course assignment but if successful the chosen company is prepared to maintain a periodic review.

There is an example of an Attitude Survey in BS8800:2004. I have used it with a degree of success.
