Exposure to second hand smoke in service user's home

I have been posed the question of what we can do to protect employees health in the event they have to go into the home of someone requiring help. This for instance could cover a health visitor, home help etc..
I am aware that a risk assessment for instance would be able to identify asthmatic employees who would be immediately at risk from second hand smoke. There is of course the potential of longer term effect of the smoke on employees.
If an employee smokes themselves, they will still be at risk from the passive smoking which could compound the effect of any damage they are doing to themselves with their own smoking habit.
How do we handle this as many people being helped in there own homes would not take kindly to being asked to stubb out.
Any advice would be welcomed here as this is indeed a difficult subject.
