Hello all
I am looking for some examples/case studies of where COSHH mismanagement occured to include in a presentation. Can anyone help?

As H&S manager for a water company - we supplied water - I was asked for a copy of our MSDS for the product we supplied to an organisation.....(in the financial sector)
After three fruitless 'phone calls to the numpty who was 'going through COSHH' I prepared the sheet. - It simply wasn't worth arguing with him.
The usual stupidities...
Protective clothing - either swim trunks/bikini or gore-tex oversuit and rubber gloves.
Risks - from inhalation - at a concentration of 1gm/cm3
Avoid prolonged skin exposure (wrinkles etc...
Mismanagement or just a numpty doing his usual best to raise the general levels of irritation in the world? I never could decide.

There was an incident - six years ago - with the Imperial College London. The consulant completed a assessment and persons may have been exposed - the works involved a fume cupboard.
The consultant was prosecuted - belive under COSHH along with the college.

Also why not do a search of the HSE prosecutions database for convictions under COSHH ?
