Posted By Ross Stirton Colleagues, I have lost the URL to the interactive website which the HSE/LA use(d) to train inspectors in how to respond to an incident. The site ran through a scenario and provided options that the user selected to indicate how they would respond. Can any of you provide the URL? Many thanks, Ross

Posted By Sean Fraser Excellent site - gives a good understanding of how and why Inspectors do what they do and is a good refresher for others involved in incident investigation to compare own training, skills and experience in evaluating and deciding on actions.

Posted By Nikki Yes a useful site, used to train LA inspectors though, not HSE inspectors. Be reassured that HSE inspectors training is more in-depth than that. Also should be taken with a pinch of salt as to what HSE inspectors actually d0 during an investigation. If inspectors did that thorough an investigation everytime there was an accident then they wouldn't get their job done. Before wasting too much resources on specialist reports etc etc them evaluate whether it is actually worth it in the long term. The main point of an accident investigation is to determine what went wrong so that you can take measures to prevent it happening again. I've known fatal accidents that require very little change in working practices becuase believe it or not accidents do happen. You can't eliminate risk. Yet I've known more minor accidents that have shown a far more significant problems with underlying safety isssues. What I'm trying to say is don't let the seriousness of the injuries blur your judgement on the seriousness of the health and safety failing.
