I have every respect for Brian - we are both among the newly appointed IOSH Trustees - but as I've said previously on this Forum I don't think this is now a very helpful document, though at the time it probably represented current thinking. If you need it for research purposes, TSHP way have a copy, or you could contact me direct - I'll have to scan the 8 pages, rather than fax it. If you wanted it for practical guidance, I believe there are better documents now. HSE's "5 steps" is a starting point, there's a free Step Change Task Risk Assessment guide on their website which covers the same area as Brian, and more, and Tony Boyle's book on Risk Management published by IOSH if you want a very thorough and robust reference.
