Can someone please point me in the right direction of how to find a copy of the near miss/ fatality triangle. With the fatality at the top and near misses at the bottom.
Thanks

Hello Brian,
Just a thought, would it be a good idea to have a 'Near Miss' Triangle sticker/label placed in the area where it occurred?

Brian,
The three accident/ near miss triangles I have come across are Heinrich, Bird and Tye/ Pearson. Heinrich's seems to be the more advanced.
