Posted By John Donaldson Several colleagues in the HE Sector have recently reported failures of the bursting disk on CO2 gas cylinders allowing the contents to discharge.
Other than when a CO2 cylinder had been involved in a fire or subject to abuse it is not something I had come across in normal use.
I would be interested if anyone else out there has experienced this problem, say during the past year.
Thanks
John

Check for commonality of manufacturer first, be careful this does not mean supplier as there are only a few companies producing these extinguishers on an outsource basis for others and parts are often common across suppliers. My brother-in-law manufactures similar discs for a number of companies. You may find that there is a batch/manufacturing problem.
Secondly - are you sure that abuse is not occurring your maintenance engineer for the extinguishers should be able to give some pointers.
Overheating might just be possible but this would suggest poorly located extinguishers.

Posted By John Donaldson Sorry I should have made it clear that these are VK cylinders holding 34 Kg of CO2.
They are commonly used in research and other areas in the HE Sector.

Omit the FE references then. I think you probably have a batch problem but you will need to research the matter in more detail.
