Posted By Kevin Forbes Back to the original question
We had an inncident at work where a hydraulic beam was being lifted and the pad eye broke causing the load to fall about 5 metres to the ground. When investigating the incident I initialy thought it would be reportable under RIDDOR (failure of lifting equipment) However It turned out that the beam being lifted was the cause of the incident and not the actual lifting equipment.
Although the beam had pad eyes these are covered under PUWER and not under LOLER
Moral of the story is if it aint covered by LOLER then it will be by PUWER
