Hi All,
I am making a presentation to Directors and Managers of my company, with the main aim, trying to get them to understand and take seriously their legal responsibilities. I would like to include some examples of prosecutions involving various levels of management, so if anybody has any such information or would know where I could find it, other than the HSE website, it would be much appreciated.
Many Thanks,
Scott

The Bakery one is always an excellent wake up call for Directors and senior managers.
Leicester bakers ordered to pay £628,000 after double death
A Leicestershire Bakery was ordered to pay £628,000 after two workers died of heat exposure as they worked on a bread oven at the company's factory on 16 May 1998. Fresha Bakeries Ltd, trading as Harvestime Ltd, and two directors and a manager pleaded guilty to eight health and safety charges. They were fined a total of £373,000 and ordered to pay costs of £255,000.
Speaking after the case, Timothy Walker, Director General of the HSE said: "I hope that today's penalties will send the strongest message to employers throughout Great Britain that protecting the health and safety of workers is a vital task for directors and managers, not an administrative

There was a good one in SHP (October 2005)
Overview from the SHP.
Company Director Jailed
A company director began a 12month jail sentance last month for manslaughter after Norwich Crown Court heard that a 36-year-old was killed on 22 December 2003 when he became trapped in a paper shredding machine.
The deceased died when he climbed into a paper shredding machine to clear a blockage, when it started - fatally injuring him.
The court heard that it became regular practice for workers to climb into the machine to clear any jams. The door of the machine's control panel had been left open - meaning the accumulation of dust and debris caused the contacts not to work on the emergency stop button, which had been wired out.
The HSE invetigation found that the machine had not been securely isolated while unblocking was carried out.
Hope this helps or failing that get any copy of the SHP

See their 'Work at height ' video or you can buy the CD for about £15.00...relates to and reconstructs a court case against the company and manager involved in a fall resulting in death of contractor.
There's also a self test section to each part of the video, with a print off certificate.
