Sorry, but I trace it back to the original Du Pont "Take Two". Probalably still available from DP in Mechelen, Belgium.
Take two seconds, two minutes, two hours, two days to prepare safely for whatever job you have to do.
T.A.K.E. 2
Tools : do I have the tools and equipement needed to do the job safely
Access : Do I have the safe means of access to the job site ; ladder, scaffolding, walkways ...
Knowledge. do I know what I have to do and how to do it
Emergency : what do I do if something goes wrong ?
2 ? Do they give me those 2 seconds or those two minutes to think/prepare before I throw myself into that black hole ?
Extremely good program, but difficult to translate from the american razz-a-matazz into any other language/culture.
just my opinion and what do I know about anything.

I don't doubt though that variations have been floating about for a number of years in the petrochem. industry. The process is also used as a supervisory or inter-employee prompt when a situation of increased risk caused by another person is witnessed and gently challenged - ie a situation of interdependency in action.
Unfortunately Laing O'Rourke have taken the same title for their "safety standowns" - the picture in the nomenclature arena is thus becoming a bit muddier.

The Take 5 "Engage the brains before the hands" programme was adopted by the oil industry in at least Scotland.
1 Stop step back observe
2 Think through task
3 Identify Hazards
4 Control Hazards & Communicate
5 Saefly Complete Task.

You maybe referring to the ‘U take 5’ program.
Five Steps:
1. stop and look
2. thing through the task
3. identify hazards
4. control and communicate
5. do the task safely (can’t be done safely stop, and contact your supervisor immediately)
In the series of books there are 3 which are all essentially targeted at empowering the employee.
The two other books are for hazobs and a STARR (safety training and risk recognition).

The company I work for introduced a scheme a few years ago (they were not the originators) called 5 x 5. The idea being to take five steps back from the job (obviously not when working at height) and take five minutes to assess the worksite and identify any hazards etc. I still have acopy of the prompt card, which I will send on to you directly.
