Posted By tip pugh can anyone help me out please, We operate a Permit to Work Procedure. We produce M/S & R/A we also submit a register for all personnel working under the M/S & R/A to sign after they have read them. Am I correct in saying that we also need documented proof that personnel have also seen and read the permit. and if that permit is ongoing and eventually gets re writen do the personnel working under that continued permit need to read and sign the new one to say that they have seen it.

Posted By s-j-o Hi Tip, what sort of industry are you in? I'm from the power industry, which has a very strict Permit to Work system. The person doing the work supplies the method statement & risk assessment. We will determine what is needed to make it safe to do the work then issue the appropriate permit.
A permit is issued to the person doing the work, or in charge of the working party. Their signature indicates that they have read the permit and understand what precautions have been taken to make the work area safe. When work is finished the permit is cancelled. If the work is not finished, the permit is surrendered & safety precautions remain in place. The permit can then be re-issued.at a later date or time allowing the job to be completed.

Posted By Philip McAleenan In addition to the previous respondee's comments, the permit receiver is charged with the responsibility of explaining the requirements of the permit to all those who are authorised and named on the permit. He must satisfy himself that they fully understand what is required of them before proceeding. This is the point when any questions or concerns are raised.
A good practice is also to post a copy of the permit at the point of work or, if this is not practicable, to have a copy available at the point of work.
