Hello,

I am currently updating our Vehicle accident procedures and vehicle accident investigation form, and would appreciate examples utilised in other areas / services.

If you could help it would greatly appreciated.

Don't know if it will be of any help to you, but I find that "HSG 245 Investigating Accidents and Incidents - A Workbook for Employers, Unions, Safety Representatives and Safety Professionals" is very informative.

Depends on the "accident". This was covered in a previous thread in 2005 and went into lenghty detail of who was and was not party to information, especially involving RTAs and lead investigations.
