I have developed a short training session aimed at mangaers and their resposibilities for health and safety. As a closing exercise i have been searching for some accident/incident scenarios that i can use.

The aim of the exercise is to promote an investigation process but be simple and relatively short.

Has anyone got a scenario with suggested outcomes/action they are willing to share. The managers are from a healthcare establishment but simple slip, trip and fall scenarios will be adequate.

Hi Mike

Pick one out of the HSE "Prosecutions" database - anonimise it but but let them know that it really is a real case and tell them the consequences [fines etc]! If that doesn't get their attention, nothing will.

I would have thought the unfolding evidence coming from the Tebay prosecution would provide a lot of issues to consider.

Safety Management Systems

Safe systems of work at work sites.

There is a video called 'No going back', which is an excellent training aid in that it demonstrates how a managers direct and indirect actions can lead to an accident.
