I am looking for a risk assessment for the identification and removal of asbestos and working in confined spaces - can anyone help? I had these with lots of other documentation on a memory stick which I seem to have lost and now require this information as a matter of urgency.

You are mixing up the two.
MDHS 100 deals with identification of asbestos during surveys and this document is available on line.
However, risk assessments for asbestos removal is not a science and although there are many ACOPS out there to give you guidance, it is the job of the asbestos removal company to give risk assessments within their ASB5 (Notification to the HSE).
Try contacting a few asbestos companies and asking them for old ASB5 documents they have issued to the HSE?
Hope this helps

Not quite correct.
An FOD ASB 5 is the notification which a licenced contractor has to submit to the HSE / LA a least 14 days before starting work on site, this is a condition of his licence.
It is a one page document and can be downloaded from the HSE www.
A site Specific method statement has to accompany this ASB for each and every Job, this is also a condition of his licence.
Both the ASB5 and the MS must be available on site for inspection by the relevant authorities
Generic MS are lodged with the HSE for all Asi Contrators which cover routine tasks which happen on site.
Email me if you want more info!
