I am looking for a risk assessment or guidance on how to document slips and trips on a cobbled stone courtyard within a school which is a listed building also. Thanks in advance Scott

What do you do for other floor types? The process I suggest is no different but you need to understand how the cobbles can cause trips. You may need to put up uneven surface signs. What is the court yard used for, is a main route, or, a looks nice bit? You will need to take account of all aspects of the use and volume of use in your risk assessment. It's simply a matter of judgement and practical experience.
